Lloyd was 98 years old and was in short term care at Koda Living Community after he had taken his second fall since November. Koda Living Community made the referral to Healthy Seniors of Steele County. Our Administrative office is now located at Koda Living Community at the Medical Complex.
We met with Becky Dawley, Lloyd’s daughter, she is old enough to retire and when we met the first week in January, she was in tears and under a high level of stress with Jury duty and her position at Federated Insurance. She was contemplating on retiring as she did not like the options available to her father in wanting to live in their home. Lloyd has limited vision and is partially blind. Lloyd was in need of respite care, and was given exercises to help reduce his risk of falling as part of his care plan once he was able to return home the second week in January. Lloyd turned 99 years young two weeks later. We started respite care and exercises each week to support Becky, his daughter, who is his primary caregiver. This support is helping her continue to stay in her career, and helping Lloyd continue to live in his home. Early March, Becky had been promoted to the level of Executive and named a Vice President, she is one of three women within this company’s history to be promoted at this level.
HSSC staff collaborates with staff, nurses, social workers from Allina Health System, Mayo Health Clinic of Owatonna, and Koda Living Community. A variety of requests from talking with seniors and caregivers that are transitioning home, signing Health Care Directives, doctors and nurses making referrals to our program and I meet with the senior or caregiver. We have met with Emergency room staff to promoting our Fall prevention education and home safety assessments to seniors that have come into emergency room from a fall. We also promote the Chronic Conditions, Live Well and a Matter of Balance workshops to our seniors. Mayo Clinic nurse that conducts Medicare Health and Wellness assessment we also cross promote our services in our intakes and newsletter. We also provide transportation for a senior who cannot drive in order to get to dialysis each week from Meridan township.
